Member's Record (MR)

The Member's permanent record in PERAA which is a requirement for issuance of ID card. Only employees included in the PRL should accomplish this form.

Member's Change or Addition Report Form (CF)

The form is used in reporting correction(s), change(s), addition(s), in connection with date of birth, beneficiaries, civil status or name previously reported in the Member's Record (MR).

Initial data sheet for institutions intending to join PERAA

Premium Remittance List (PRL)

The list of employees and corresponding retirement premiums for the month/period paid for.

Loan Remittance List (LRL)

The list of employees and corresponding loan amortization for the period paid for.
